Each CHPA is required to re-certify every three years. The fee for recertification is $100.00.

For a copy of the CHPA renewal application form click either (Word) or (PDF) to download in the format of your choice. Completed forms and all appropriate documentation should be sent to:

CHPA Re-Certification Criteria

CHPA must accumulate a total of fifteen credits by the end of the third year. The credits are attained from the five general categories listed below:

Education	Maximum of 3 credits allowed
Experience	Maximum of 5 credits allowed
Membership	Maximum of 5 credits allowed
Specialized Approved Training	Maximum of 5 credits allowed
Professional Healthcare Security / Safety / Risk Management Activities or Accomplishments	Maximum of 7 credits allowed
